Offered by St. Andrew’s Community Hospital, Community Care Services provides integrated and holistic care to support those who are home-bound.

Services Provided

Home Medical

Primary care, chronic diseases management

Home Nursing

Nursing care procedures, medication management, continence care, wound care, nutrition counselling and education, caregiver training (pertaining to the home-bound’s needs)

Home Therapy

Occupational therapy, physiotherapy, speech therapy, caregiver training (pertaining to patient’s needs)

Inter-generational bonding over gardening
Volunteers accompanying the seniors for
supermarket shopping

Eligibility and Admission

Adult patients who are frail with chronic illnesses or with long-term clinical care needs and are home-bound and/or bed-bound and/or have a physical constraint in accessing clinical care.

Eligibility criteria

  • Adults with medical conditions who require continuing care but are unable to go to any healthcare facilities due to their physical condition.
  • Admission is through referral from the Agency for Integrated Care (AIC) only.
  • HCS is offered to patients living in the eastern part of Singapore by referrals through the AIC only.
